This is a chance to help make a difference by opening your home to an individual with developmental and/or physical disabilities. Open your home to someone with a disability! We are seeking someone in the Cookeville, TN area who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. This independently contracted position will provide a 24/7 family-like setting for an individual. This role will help care for an individual in need of assistance with medical appointments and minor assistance with grooming.

REQUIREMENTS:

Must be comfortable living with a cat High School Diploma/GED Valid Driver's License Previous experience working as a Direct Support Professional is required Will need to successfully complete trainings through Community Options Must be open to supervisory visits in your home Our Family Model Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
